Cardiac Education and Rehabilitation Program – Patient Education Workbook

You may be joining our Cardiac Education and Rehabilitation Program (Cardiac Rehab Program) because you’ve suffered a heart event (e.g., heart attack, stroke) or you’ve had a heart procedure (e.g., stent, bypass surgery). You may also be here because you’re at risk of experiencing a heart event in the future (e.g., chest pains).

While we provide our clients with a hardcopy of the Patient Education Workbook, we are happy to offer this as a digital resource if you want to review content online or print off any content.
