SLMHC, SLFNHA partner to provide regional heart failure care

Sioux Lookout Meno Ya Win Health Centre (SLMHC) and Sioux Lookout First Nations Health Authority (SLFNHA) have partnered with the Peter Munk Cardiac Centre at University Health Network (UHN) to provide heart failure care in the region and implement Medly, a digital health program that monitors your heart failure symptoms and connects you to the care team remotely.

The UHN Cardiology team aims to offer in-person heart function clinics in Sioux Lookout and Sandy Lake First Nation three times per year. Eligible heart failure patients will be also enrolled onto Medly and managed remotely. Virtual appointments can also be scheduled.

Heart failure is a common condition where the heart is unable to meet the demands of the body. It does not mean that the heart has stopped working or is beyond repair. Many treatments are available to help patients live longer and better.  Medly helps patients self-manage heart failure, increases communication with their healthcare team, and improves the way doctors manage their condition. This program is not a replacement for the care they receive, but it does track patient measures and sends the information to the Medly Coordinator and doctor.
