SLMHC, SLFNHA partner to provide regional heart failure care
Sioux Lookout Meno Ya Win Health Centre (SLMHC) and Sioux Lookout First Nations Health Authority (SLFNHA) have partnered with the Peter Munk Cardiac Centre at University Health Network (UHN) to provide heart failure care in the region and implement Medly, a digital health program that monitors your heart failure symptoms and connects you to the care team remotely.
