Heart Function Program
SLMHC and SLFNHA have partnered with the Peter Munk Cardiac Centre at University Health Network (UHN) to provide heart failure care in the region and implement Medly, a digital health program that monitors your heart failure symptoms and connects you to the care team remotely.
The UHN Cardiology team aims to offer in-person heart function clinics in Sioux Lookout and Sandy Lake First Nation three times per year. Eligible heart failure patients will be also enrolled onto Medly and managed remotely. Virtual appointments can also be scheduled. Primary care providers across the north west region can refer patients to the UHN heart function clinics via this referral form.
What is Heart Failure?
- Heart failure is a condition where the heart is unable to meet the demands of the body. It does not mean that the heart has stopped working or is beyond repair.
- Heart failure is a common condition.
- Many treatments are available to help patients live longer and better.
- In general, heart failure can be treated, but not cured.
- More information on treatments, prevention and managing heart disease at: www.ourhearthub.ca
What is Medly?
- Medly helps you self-manage your heart failure, increases communication between you and your healthcare team, and improves the way your doctors manage your condition. This program is not a replacement for the care you are currently receiving.
- Medly uses a smartphone application where you record your weight, blood pressure, heart rate, and symptoms.
- You can track your measures and how you are doing. Medly Coordinator and doctor also receive the information. Medly gives you automated feedback based on targets set specifically for you by your healthcare team.
- You can also chat via Medly with your Medly Coordinator to discuss how you are feeling or the instructions provided by the program.
- To use Medly, you need a smartphone, weight scale, and blood pressure monitor. If you do not have one of these pieces of equipment, we can provide you the needed equipment.

The clinic will see patients with:
- Chronic complex medical conditions
- Uncontrolled diabetes
- Hypertension
- COPD
- Not Yet Diagnosed (NYD) acute medical conditions that are stable enough to not needed the emergency room

- Common internal medicine procedures (skin biopsies, thoracentesis, paracentesis, diagnostic lumbar puncture, joint injections/aspirations)
The Vascular Surgery Service
Based out of Thunder Bay Regional Health Sciences Centre (TBRHSC), the Vascular Surgery Service provides care to all of Northwest Ontario. Vascular Surgery treats veins and arteries all over the body, with the exception of heart blood vessels (treated by cardiologists and cardiac surgeons) and vessels within the brain (treated by neurosurgeons).
We manage aortic disease such as aneurysms, arterial disease such as blockages, vein disease such as venous ulcers, and provide vascular care for trauma and dialysis access. We do assessments at TBRHSC, Sioux Lookout Meno Ya Win Health Centre (SLMHC), and Saint Joseph’s Hospital as well as via Telehealth. Procedures are currently done only at TBRHSC, so patients who require vascular angioplasty or surgery would travel to Thunder Bay.
The Vascular Surgery service runs a two-day clinic every Spring and Fall at the SLMHC Specialty Clinic. Patients travel from Northern communities and surrounding area as far away as Kenora. Patients may have their imaging done either at their home hospital or in Sioux Lookout and will be seen at SLMHC by one of the Vascular Surgeons (Dr. MacDonald), usually in late April and late September.
