Registered Practical Nurse – Coordinator of Extended Care

Full Time (Non-Union)

Reports To: Patient Care Manager – LTC

Job Summary:

The Sioux Lookout Meno Ya Win Health Centre is seeking a licensed RPN to fill the roles as the Coordinator of Extended Care. The COEC will be accountable to the Director of Care for the day to day management of the William A. George Extended Care Unit and resident care services. The COEC will ensure the delivery of optimal care and services.

 
Qualifications

  • Current certificate of competence with the College of Nurses of Ontario, as a licensed Registered Practical Nurse required
  • Minimum 4 years combined experience in LTC facility nursing and acute care setting with demonstrated leadership, assessment, and time management skills
  • Possess current First Aid/CPR certificate
  • Experience in geriatric, rehabilitation, or long-term care nursing is desirable
  • Must have knowledge of the nursing process
  • Post-graduate studies in gerontology or related field is desirable
  • Must have good leadership and communication skills
  • Possession of skill in planning, coordination, and supervising in a comprehensive nursing care program
  • IV certified and experienced
  • RAI Coordinator certification preferred
  • Advanced Foot and Wound care preferred
  • Current GPA, PIECES, LEAP an asset
  • Required to be fully vaccinated in accordance with the Ministry of Health vaccination guidelines
Job Duties:

  • Coordinates and completes RAI assessments and reporting as required
  • Assess, plan, and provide interventions, utilizing the current Standards of Nursing Practice, CNO, and the Long-Term Care Legislation
  • Acts as a liaison between Manager or Director of Care and the care team
  • Acts as a clinical resource to the on-duty care team as necessary
  • Lead new initiatives and facilitate continuing education to the care team as necessary
  • Advocate for resident and family centered care
  • Advocate for a Healthy Workplace Environment
  • Excellent computer skills, including use of Microsoft Word, Excel, and RAI
  • Excellent verbal and written communication skills
  • Ability to work with peers in a team approach
  • Must be physically capable of performing duties in this position
  • Must be willing to work assigned shifts (8 hour rotation shifts)
  • Understanding the First Nation culture and the ability to speak Oji-Cree is a definite asset.

 

Salary:$42.56 - $44.62 (Salary and Benefits in accordance with qualifications and experience)
