Social Worker – Psychiatry – MHAP 01-24
Job Summary: Reporting to the Manager of Mental Health and Addictions Program (MHAP), Assault Care and Treatment Program (ACT), Sioux Lookout Diabetes Programs (SLDP/CCDC), the Social Worker is responsible for coordinating, developing, implementing and evaluating case management services associated with that program. Key responsibilities include: facilitating screening and assessment in conjunction with the psychiatrist; providing case management services for the program clients as needed; and promoting community linkages related to the identified target population. In consultation with the patient, family/significant others and the health care team, individualizes the multidisciplinary patient care path to reflect interventions to meet identified needs while promoting cultural appropriateness, safety, privacy and confidentiality for patient/family/significant others.
Qualifications:
- Post-secondary education in Social Service Work, Social Work, or equivalent
- Registration with Ontario College of Social Workers and Social Service Workers, or eligible to obtain.
- Innovative thinker, able to work in a result focused environment.
- Ability to analyze and interpret the needs of clients and offer the appropriate options, solutions, and resolutions
- Excellent skills in all MS Office.
- Experience in healthcare organization is an asset.
- Exceptional conflict resolution and negotiation skills.
- Able to respond quickly in a dynamic and changing environment
- Highly flexible, with solid interpersonal skills that allow one to work effectively within an interdisciplinary team, and with patients, families and communities
- Training or experience in complex mental health needs including psychiatric, cognitive behavioural therapy, crisis or suicide intervention, and/or motivational interviewing is an asset.
- Able to build and maintain lasting relationships with other departments, communities, fellow employees and patients.
Job Duties:
- Assess and prioritize patient needs, establishing individualized plan in collaboration with patients and families.
- Works cooperatively and collaboratively with other members of the circle of care.
- Provide services that are individualized and tailored to address patient’s current needs and preferences.
- Maintains an updated knowledge of services provided by community resource agencies.
- Maintains accurate patient care documentation, case management and statistics in accordance with hospital standards.
- Advocate on behalf of patients.
- Acts as a navigator for regional psychiatry services and to direct patients to sources of support by assisting with completion of social assistance applications and support letters.
- Assist children and families to develop healthy coping skills.
- Provide crisis intervention and appropriate counselling or therapy services.
- Maintain communication with patients and the interdisciplinary healthcare team.
