Patient Navigator (IPP)

Department: Integrated Pregnancy Program
Reports to: Manager of Mental Health and Addictions Program (MHAP), Assault Care and Treatment Program (ACT), Sioux Lookout Diabetes Programs (SLDP/CCDC)
The Patient Navigator will work closely with the interdisciplinary team within the Integrated Pregnancy Program (IPP). The Patient Navigator provides assistance and support services for people in the Integrated Pregnancy Program. Following the Mission, Vision, and Values of Sioux Lookout Meno Ya Win Health Centre will enable the Patient Navigator to assist clients with anything that falls under their social determinants of health. The Patient Navigator will initiate referral to appropriate personnel based on knowledge of the scopes of practice of other members of the health care team/advocates on behalf of patients and family/significant others. In consultation with the patient, family/significant others and the health care team, individualizes the multidisciplinary patient care path to reflect interventions to meet identified needs while promoting cultural appropriateness, safety, privacy and confidentiality for patient/family/significant others. The Patient Navigator will perform efficient coordination and navigation of all healthcare appointments.
• Minimum grade 12 education required
• Experience as a patient navigator preferred
• Experience assisting patients to navigate Integrated Pregnancy Program
• Innovative thinker, able to use and develop new sources for recruitment
• Able to work in a result focused environment
• Ability to analyze and interpret the needs of clients and offer the appropriate options, solutions, and resolutions required
• Exceptional conflict resolution, negotiation, and objection handling skills
• Able to respond quickly in a dynamic and changing environment
• Highly flexible, with solid interpersonal skills that allow one to work effectively within an interdisciplinary team, and with patients, families and communities
• Excellent training, coaching, and active listening skills are essential
• Able to build and maintain lasting relationships with other departments, communities, fellow employees and patients
• Excellent skills in all MS Office (focus on Excel)
• Ability to work under pressure to meet departmental targets
• Ability to work well independently.
• Knowledge and sensitivity to First Nations language and culture an asset.
• Experience working in a case management model of care an asset.
• Ability to speak Ojibway/Oji-Cree/Cree an asset
• Case management services to the patients enrolled in the Integrated Pregnancy Program.
• Assess and identify socioeconomic individual needs and assist patients to improve functioning with reduced symptomology.
• Provide patient services to Integrated Pregnancy Program (IPP) patients in coordination with IPP patient appointments
• Works cooperatively and collaboratively with other members of the team.
• Provide services that are individualized and tailored to address patient’s current needs and preferences
• Maintains an updated knowledge of services provided by community resource agencies
• Maintains accurate patient care documentation, case management and statistics in accordance with hospital standards
• Acts as a navigator to direct patients to sources of financial, administrative and cultural support by assisting with completion of applications, support letters and referrals
• Acts as support and a navigator in providing referral options for counselling and therapy.
• Maintain communication with patients and the healthcare team
