ALC Coordinator (NADMIN 06-25)
Job Summary:
The ALC Coordinator plays a crucial role in enhancing patient care on the ALC and Med/surge Nursing floors as well as in the Emergency Department at Sioux Lookout Meno Ya Win Health Centre. This position involves close collaboration with an interdisciplinary team to provide support and assistance to clients identified as Alternate Level of Care (ALC) patients. The ALC Coordinator is dedicated to addressing the social determinants of health impacting patients, with a specific focus on securing alternative housing options. The ALC Coordinator is responsible for ensuring that the organization meets the Home First requirements, facilitating a timely and appropriate transition for ALC patients out of the hospital.
Qualifications/Experience:
• Experience working in health care or health-related field.
• Association in registration with governing body considered an asset.
• Minimum (1) year experience in health-related field.
• Demonstrated knowledge and practical experience in working with community organizations and resources.
• Demonstrated knowledge and practical experience in working with an Interdisciplinary team.
• Demonstrated knowledge and practical experience with patient flow.
• Excellent interpersonal and leadership skills.
• Excellent communication skills both written and verbal.
• Ability to work well independently.
• Knowledge and sensitivity to First Nations language and culture an asset.
• Experience working in a case management model of care an asset.
• Ability to speak Ojibway/Oji-Cree/Cree an asset.
Job Duties:
• Collaborate with healthcare professionals on the Med/Surg nursing floor and in the
Emergency Department to identify and support ALC patients.
• Provide assistance to clients in addressing their social determinants of health, including but not limited to housing, financial support, and community resources.
• Actively search for and apply for alternative living accommodations for patients
transitioning out of the hospital.
• Work closely with community health leaders and local organizations to facilitate access to community resources and housing options for patients.
• Monitor and evaluate the effectiveness of support services provided to ALC patients, ensuring alignment with the Mission, Vision, and Values of Sioux Lookout Meno Ya Win Health Centre.
• Maintain accurate and confidential records of patient interactions and progress in
securing alternative living arrangements.
• Educate patients and their families about available resources and assist them in
navigating the healthcare and housing systems.
• Work to facilitate patient flow within the organization.
• Participate in quality improvement strategies and initiatives.
• Works cooperatively and collaboratively with other members of the team to provide
services that are individualized to the client and family.
• Must have the ability to work with minimal supervision.
• Maintain and submit monthly report related to ALC activities.
